A new service for family caregivers of patients with Alzheimer’s disease.

If your loved one with Alzheimer’s Disease is living in Florida, you may be eligible for a free NAN subscription.

 
 

The Florida Department of Elder Affairs (DOEA) has joined forces with Navigating Aging Needs, LLC (NAN) to reduce the burden of care for family caregivers of older adults with Alzheimer’s Disease.

“We are thrilled to be partnering with the Florida Department of Elder Affairs to offer this evidence-based model of care to Floridians who are dealing with the medical, emotional, and financial impact of Alzheimer’s,” says Rosemary Laird MD, founder of NAN.

 

As a NAN subscriber you will be assigned an exclusive NAN Navigator, a dedicated expert to assess your needs and the needs of your loved one. Your personal NAN Navigator will develop a NAN plan and will guide you to resources that will address any medical, emotional, social, financial, or legal challenges you are facing. The NAN Navigator will be your ongoing resource throughout the subscription period, providing information, coaching, and emotional support, all of which leads to improved outcomes for both you and your loved one.
